Info for Your 1st Visit to Lafayette & San Ramon Acupuncture
Congratulations on your decision to try acupuncture! You’ll find acupuncture to be a rewarding experience on many levels. My needles are thin; my style is gentle, and you will feel better as your body comes back into balance. In addition to a decrease in pain, you should also feel more relaxed, energetic and at peace with the world.
Please download, print, and complete the Acupuncture Intake Form and bring it to your first appointment. If you’d like to complete it at my office, please arrive 15 minutes early to do so. If you’d like to review my office’s HIPPA policies, you can download our NOTICE OF PRIVACY PRACTICES.
As a new patient, you’ll be scheduled for a 1.5-hour session. Because acupuncture is a holistic medicine, I will conduct a thorough verbal intake, covering not just your chief complaint, but other aspects of your health and wellness. I will answer any questions you have along the way.
You can expect an acupuncture physical exam that includes my looking at your tongue, feeling your pulse, pressing injured areas for pain and discomfort, and pressing different locations on your stomach. I will also measure your blood pressure and heart rate.
Based on the information I gather, I’ll formulate an acupuncture/Chinese medical diagnosis and treatment plan. The plan will include a prognosis that describes the improvement I would expect for your condition after a specified number of acupuncture treatments, along with possible herbal formulas, and nutritional and/or lifestyle recommendations. I will explain this to you in detail and answer any questions you may have during and after the acupuncture treatment.
To get the most out of your initial acupuncture session, please do the following:
- Download and complete this Acupuncture Intake Form, and bring it with you to your appointment
- Show up on time, with food in your stomach
- Wear loose, comfortable clothes that can be easily rolled up past the knees and elbows
- Discuss any/all of your questions and concerns
- Refrain from heavy exercise, drugs or alcohol for at least six hours after treatment
- Follow your treatment plan

If you are unable to get your health insurance to cover acupuncture services, you can purchase acupuncture treatment packages that offer significant savings. There is no time limit on using your package and you can get a refund at any time.
The fine print: For the Acupuncture Treatment Package of 6 Returning Acupuncture Sessions for the price of 5, the first 5 treatments are valued at full price ($90), and the 6th session is complimentary. Refunds are prorated by the number of treatments out of 5 that have been provided.
For the Acupuncture Treatment Package of 10 Returning Acupuncture Sessions for the price of 8, the first 8 treatments are valued at full price ($90), and the 9th & 10th sessions are complimentary. Refunds are prorated by the number of treatments out of 8 that have been provided.
As a courtesy to wait-listed patients, if you need to change or cancel your acupuncture appointment, please give at least a 24-hour advance notice. Otherwise the full price of your appointment will be charged.
Please be on time. If you know that you will be late, please call. Every effort will be made to reschedule you for a later time. If an emergency prevents you from keeping your appointment, arrangements can be made. Fees for missed appointments are NOT covered by insurance.
Many healthcare insurance plans have acupuncture as a covered modality with various ranges of reimbursement. I am happy to bill your insurance directly for acupuncture related procedures and appropriate diagnoses if you have the right type of acupuncture insurance coverage.

I am also a member of the City and County of San Francisco Medical Provider Network that services city employees.
Acupuncture insurance coverage depends on your individual plan. My office can verify your acupuncture benefits if you send me an email with the following information:
- Copy of the front and back of your health insurance card (scan or photo, or fax to 925-403-1001)
- Your Full Name, Home Address, Date of Birth, Marital Status, and Employment Status
- If the health plan is through your spouse, their Full Name and Date of Birth
- The health condition for which you’d like to get treated
I will contact you within 2 working days to verify benefits.
Workers’ Comp cases are accepted with proper authorization. I treat patients as a physician within the Workers’ Comp system working with your Primary Treating Provider. I do not manage cases as a Primary Treating Provider.
I am also a member of the City and County of San Francisco Medical Provider Network that services city employees’ Workers’ Comp cases.
I accept Personal Injury cases with proper documentation.
